Good Faith Estimate Notice

You have the right to receive a Good Faith Estimate that explains how much your medical and mental health care will likely cost.

Under federal law, health care providers and facilities are required to provide a Good Faith Estimate (GFE) of expected charges to uninsured or self-pay patients (those not using insurance) for items and services provided. This includes mental health services such as psychotherapy, counseling, or medication management.

What You Can Expect:

You have the right to receive a Good Faith Estimate in writing at least 1 business day before your scheduled service if you request or schedule care at least 3 days in advance.

The estimate will include expected costs of services and any related fees (such as facility, lab, or testing fees, if applicable).

You can also request a Good Faith Estimate at any time before scheduling a service, even if you haven’t set an appointment yet.

Disputing a Bill:

If you are billed more than $400 above the amount listed in your Good Faith Estimate, you have the right to initiate a dispute resolution process through the U.S. Department of Health and Human Services (HHS).

To protect your rights:

Keep a copy or take a screenshot of your Good Faith Estimate.

Save all related documents and receipts.
